What is Case Management?
Cal State East Bay's Case Manager. Lizeth Cortez, coordinates support efforts for students facing emergencies, financial challenges, and other barriers to their success.
Role of Case Manager:
- Assess student needs
- Engage students of concern
- Develop a plan of action tailored for each student
- Assist students in accessing services, both on and off-campus
- Work with student to build or strengthen their support systems
- Advocate for students individually and systemically
- Remove obstacles to getting help
- Foster self-advocacy in students to manage their academic, personal and fiscal responsibilities
- Follow up with student to help them stay on track toward success
- Arrange for appropriate referrals/services
How to Identify a Student of Concern:
Some signs that a student may be experiencing challenges to their success include:
Physical Signs
- Poor personal hygiene
- Change in appetite; weight loss or gain
- Agitation or restlessness
- Recurring physical complaints
Psychological Signs
- Delusions or paranoia
- Guilt or worthlessness
- Sad, anxious, empty mood or mood swings
Change In Behavior
- Withdrawal or isolation
- Loss of interest or pleasure in activities
- Increased alcohol or substance use
- Irritability or anger
- Increased absences or late arrival to class/work
Unmet Basic Needs
- Housing insecurity
- Food insecurity
- Financial struggles
- Medical Services
What do I do if a student’s behavior is concerning?
- Submit a CARE Report to alert our Student Care Team. See link below.
- The report will be assigned to the most appropriate team member for follow up.
- We will keep your identity as a reporter anonymous. However, students are often more open to this referral if you let them know that someone will be reaching out to them.
- Students will be offered an assessment and support services, but they have the right to decline.
Who can submit a Care Report?
- Any faculty, staff, student, parent, family or community member who has a concern about a student and wants to connect them to additional support.
