A Certificate of Clearance and a valid TB test is required by orientation if admitted to the program~
Getting the Certificate of Clearance is a two-part process.

Print the and take to a location offering Live Scan services to have your fingerprints rolled or scanned.

Go to and apply for the Certificate of Clearance. For more details, visit .
Once the Certificate of Clearance is issued, you can access the document from the website that displays the document name, issuance date, and expiration date. The document must remain valid while in the program.
Do NOT submit the LiveScan form.

Verification of Experience Form-Need verification of a minimum of 3 years of full-time teaching or services experience under your preliminary/clear credential. Student teaching, intern teaching, or teaching while holding an emergency or teaching permit does not count as part of the three years. Have your district HR complete the form.
